Position Summary

At Athletico, the Physical Therapist’s role is to evaluate, design and implement a patient’s plan of care. Each Plan of Care is created to increase, maintain or restore function, minimize or prevent disability, and assist patients in reaching their maximum performance levels. Duties and Responsibilities

The below is not an exhaustive list of duties and you will be expected to perform different tasks as necessitated by your changing role within the organization and the overall business objectives of the organization. Assess and screen patient's daily living/work‐related skills and develop therapeutic retraining programs with measurable Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the patient’s level of function. Confer with the patient's physician and other health care team members and participate in development/revision of the Plan of Care Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range‐of‐motion and function or teach compensation Review/expand the treatment programs in a manner consistent with the functional goals of each patient and within the guidelines of the rehabilitation program Report patient status and progress to the physician, rehabilitation staff, patient and family me Participate in care coordination and discharge planning activities and act as a resource to other health care personnel in meeting patient's Recommend and/or design special adaptive equipment for clients to improve residence or working environments or improve their participation in the rehabilitation program and/or community Train family members and caregivers to carry out the treatment including dressing, feeding, grooming and hygiene skills, participate in team and family educational meetings Accurately and thoroughly document patients' care observations, interventions and evaluation on the day services are Ensure that interim (verbal) orders received from the physician are accurately documented and implemented. Submit evaluation, treatment plans and discharge summary within twenty four (24) hours.

Qualifications

Education: Degree from an accredited Physical Therapy Program Work Experience: A minimum of 5 year physical therapy experience preferred Home Health experience preferred Certification/Licensure: Current Physical Therapist licensure in good standing Knowledge and Technical Skills: Current CPR certification Proof of a negative TB test recommended Exceptional organizational and communication skills Current and valid driver’s license Current and valid automobile insurance Language Skills: Ability to read, write and speak English proficiently Physical Demands: The physical demands and work environment described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Requirements

Candidates must hold a degree from an accredited Physical Therapy program and a valid state license. A minimum of 5 years of experience and home health expertise are preferred, along with CPR certification and a valid driver's license.
