Provide in-home and community-based support to families and individuals dealing with severe mental illness or substance use disorders. Collaborate with multidisciplinary teams to offer advocacy, referrals, and navigation of mental health and educational systems.
Requires a high school diploma or equivalent and a valid Massachusetts driver's license. Candidates must have lived experience as a caretaker for youth or adults with mental illness or substance use disorders.
Provide in-home and community-based support to youth, adults, and families assisting parents and caregivers Collaborate with the ACCS team and families to offer in-person and telephonic support, referrals, and linkage to services Support parents and caregivers through advocacy, skill-building, and navigation of mental health, educational, and community systems Ensure all services are culturally and linguistically appropriate while promoting strengths-based, person-centered, and recovery-oriented care Assist families with treatment and safety planning, crisis intervention, and connecting to local, state, and federal resources
