Manage chronic diseases for geriatric patients through medication adjustments, comprehensive reviews, and collaborative practice agreements. Lead inter-disciplinary care huddles and provide clinical pharmacy expertise to improve patient outcomes and operational efficiency.
Requires a Bachelor's or Doctor of Pharmacy degree and 2 years of relevant pharmacy experience or an ASHP residency. Must hold a Michigan Clinical Pharmacist license and BLS certification, with PGY2 residency and BC-PS certification preferred.
