The RN provides clinical nursing practice focused on telephonic patient triage, medication administration for cardiac CTs, and post-hospital discharge follow-up. They coordinate care delivery across disciplines to improve patient outcomes and decrease unnecessary healthcare utilization.
Candidates must hold a PA Registered Nurse License and BLS Certification. A Bachelor of Science in Nursing or a specialty certification is required, or enrollment in an RN to BSN/MSN program with recent credits.
Telephone triage, assist with medication administration for cardiac CTs at Spring Ridge office, assist with administration of medications in clinic, back up for anticoagulation clinic, back up as hospital RN coordinator facilitating rounding, post hospital discharge follow up and testing Provides competent clinical nursing practice across the life-span, facilitates transitions across care settings, and coordinates care delivery with other disciplines for improving patient care quality outcomes as well as decreasing unnecessary utilization and associated costs across patient populations and health care settings. Daily ambulatory nursing practice performance primarily is focused on telephonic patient triage, patient education, direct care interventions, collaborative leadership and care coordination. #READ
Education Requirements Bachelor of Science in Nursing ? OR - Certification in specialty by recognized accrediting agency ? OR ? enrolled in RN to BSN or RN to MSN program with minimum 12 credits completed within previous 12 months Completes and maintains all competencies and trainings as required for role.
