About the role

The Nurse Practitioner is responsible for assessing, diagnosing, and managing the healthcare needs of clients with autism and developmental disabilities. This includes performing physical exams, developing individualized care plans, and collaborating with a multidisciplinary team to ensure coordinated care.

Liberty Community Program is a leading organization dedicated to providing compassionate care and support for individuals with autism and developmental disabilities. We are currently seeking a skilled Nurse Practitioner to join our dynamic team. The Nurse Practitioner (NP) will play a pivotal role in assessing, diagnosing, and managing the healthcare needs of our clients. This position is essential for bridging the gap between patient needs and clinical services in a supportive and collaborative environment. The Nurse Practitioner will conduct comprehensive health assessments, develop treatment plans, and provide ongoing monitoring of chronic and acute conditions. The ideal candidate should possess a deep understanding of behavioral health and be passionate about improving the quality of life for those we serve. Key Responsibilities:

Perform thorough physical examinations and health assessments for participants.

Develop and implement individualized care plans based on assessment findings and client needs.

Diagnose and manage acute and chronic health conditions.

Prescribe medications and monitor their effects.

Provide education and support to clients and families regarding health management and wellness strategies.

Collaborate with a multidisciplinary team including physicians, therapists, and direct support staff to ensure coordinated care.

Maintain accurate and timely documentation of patient interactions and care plans.

Conduct follow-up visits to assess progress and adjust care plans as necessary.

Master’s degree in Nursing or Doctor of Nursing Practice (DNP)

Valid Nurse Practitioner (NP) certification and licensure in the state of West Virginia.

Experience in a healthcare setting, with a focus on behavioral health or developmental disabilities preferred.

Strong clinical assessment skills and ability to make independent medical decisions.

Excellent communication and interpersonal skills.

Familiarity with electronic health record (EHR) systems.

Ability to work collaboratively with a multidisciplinary team.

Valid CPR/ BLS certification.

Commitment to providing high-quality care and advocacy for individuals with disabilities.

Must have reliable transportation and the ability to travel for services as needed.

Requirements

Candidates must hold a Master's in Nursing or DNP and possess a valid Nurse Practitioner license in West Virginia. Experience in behavioral health or developmental disabilities and valid CPR/BLS certification are required or preferred.
