The LPN is responsible for providing high-quality technical skilled nursing care to patients within their homes according to a plan of care. Key duties include thorough documentation in electronic medical records and active participation in the agency's care coordination process.
Candidates must possess a current and unrestricted LPN license, CPR certification, and a valid driver's license with vehicle insurance. The ability to work flexible hours and function in diverse home environments is required.

Home care experience Ability to manage multiple tasks simultaneously Able to work independently Good communication, writing, and organizational skills Annual Target Pay - $49,151 Per Visit Point Pay Range - $23.63 Hourly Pay Range - $28.36 Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
