Summary We are seeking a highly skilled and empathetic Advanced Practice Registered Nurse (APRN) to join our telehealth team. This specialized role focuses on providing comprehensive memory assessment and care through a virtual platform. You will be responsible for conducting cognitive screenings managing the symptoms of patients living with dementia. A portion of this role involves managing the complex behavioral and psychological symptoms of dementia (BPSD), ensuring that both patients and their caregivers have a clear, medically sound path forward from the comfort of their homes. How will you make an impact & Requirements Key Responsibilities Virtual Assessments: Conduct comprehensive telehealth evaluations for patients with memory concerns, including detailed clinical histories and remote physical/neurological observations. Cognitive Screening: Administer and interpret validated remote assessment tools (e.g., Mini-Cog, MoCA, SLUMS) to determine the stage and type of cognitive impairment. Psychiatric Management: Diagnose and manage neuropsychiatric complications such as agitation, anxiety, depression, and sleep disturbances using both pharmacological and non-pharmacological interventions. Caregiver Support: Educate families on disease progression, safety planning (wandering, driving), and behavioral de-escalation techniques.
