Two years' experience required in health care. Experience includes case management/discharge planning in one of the following settings: Acute care, Home care, LTC care, Physician Office, or Managed Care company. Clinical competence in disease management and case management principles. Must possess excellent interpersonal communication and negotiation skills, problem-solving skills, strong organizational and time management skills, and the ability to work independently and as a member of the care team. Requires demonstrated knowledge and proficiency in appropriate tools. BLS required per policy guidelines.

Apply the nursing process to coordinate care, optimize patient outcomes, and ensure effective resource use. Collaborate with patients, providers, and the community to maintain continuity of care and develop care plans across settings. Conduct comprehensive assessments, align services with patient needs, and manage accurate documentation and case records. Maintain knowledge of reimbursement requirements and demonstrate leadership by adhering to professional nursing standards and evaluating personal practice.
