Schedule

We have multiple schedule variations available for this role. Whether you are looking for a weekend per diem or a full-time role, you will be able to choose your ideal schedule.

Per Diem: Saturday & Sunday 8 hour days

Friday, Saturday & Sunday or Saturday, Sunday & Monday 8 hour days

Full Time: Friday, Saturday, Sunday and Monday 8 hour days

Please note all schedules will include alternating weekend on call expectations

Who We're Seeking

Are you ready to make a meaningful difference in the lives of individuals and families facing end-of-life challenges? Our Hospice Care team is seeking a dedicated Registered Nurse to deliver personalized, one-on-one care in the comforting setting of patients' homes. Regular hours for this position are 2 - 4 days per week – a combination of Friday-Monday 8AM-5PM and acts as after hours on-call triage and runner.

Your Responsibilities

As a member of our dedicated team, you'll:

Provide compassionate end-of-life care, ensuring the physical and emotional well-being of patients and their families.

Deliver expert pain and symptom management, offering comfort and support during challenging times.

Collaborate closely with our interdisciplinary team, local physicians, and community providers to optimize the quality of life for patients.

Requirements

  • VT Licensed RN (BSN preferred)
  • Minimum of two years of clinical med-surg and acute care nursing experience required. One year of Hospice experience is preferred. Home Health experience and/or previous case management experience preferred.
  • Valid driver’s license
