Provide neurobehavioral health services, evaluations, and therapy to elderly residents in long-term care facilities. Coordinate holistic care plans with multidisciplinary teams, primary care physicians, and families.
As a Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC) with GuideStar Eldercare you will provide neurobehavioral health services to residents in long-term care facilities. Using a multi-disciplinary approach, you will partner with specialty clinicians to provide holistic, continuity of care for elderly individuals who are afflicted with neurocognitive disorders and other neurologic and/or psychiatric conditions.
