Responsibilities

Provide skilled nursing care to patients in their homes, ensuring adherence to care plans. Conduct patient assessments and monitor vital signs to evaluate health status. Administer medications and treatments as prescribed by physicians. Educate patients and families on health management and disease prevention. Collaborate with interdisciplinary teams to optimize patient care outcomes. Document patient progress and maintain accurate health records. Assist with daily living activities to enhance patient comfort and quality of life. Participate in ongoing training and professional development opportunities.

Requirements

Current Registered Nurse (RN) license in the state of Georgia. Minimum of 1 year of nursing experience, preferably in home health or community settings. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively. Basic Life Support (BLS) certification required. Compassionate demeanor with a patient-centered approach to care. Proficient in electronic health record (EHR) systems.
