Perform home visits for patients in a home care setting Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in condition Teaches, supervises, and counsels the patient and family regarding home therapy procedures and other care needs as appropriate to the patient’s condition
Meets the education and experience requirements for Certificate of Clinical Competence (CCC) in speech pathology or audiology granted by the American Speech-Language-Hearing Association, or has completed the educational requirements for certification and is in the process of accumulating the supervised experience required for certification. Is certified to practice Speech-Language Pathology. Is licensed to practice speech pathology in the state of Connecticut. Has a minimum of one (1) years’ experience in a health care facility or equivalent experience.
