Job Title: Licensed Master Social Worker (LMSW) – Part-Time / PRN
We are seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our Home Health and Hospice team on a Part-Time or PRN basis. The LMSW will provide psychosocial assessments, counseling, emotional support, resource coordination, and advocacy services to patients and families experiencing illness, disability, end-of-life care, and bereavement. The LMSW will work collaboratively with physicians, nurses, therapists, and other interdisciplinary team members to ensure high-quality patient-centered care.
Master's Degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE). Licensure
Current Licensed Master Social Worker (LMSW) license in the State of Texas. Current driver's license and reliable transportation. Current automobile liability insurance. Experience
Minimum one (1) year of social work experience preferred. Experience in Home Health, Hospice, Healthcare, Case Management, Geriatrics, Palliative Care, or Medical Social Work preferred. Knowledge of Medicare, Medicaid, and community resources preferred. Skills and Abilities
Strong assessment, counseling, and crisis intervention skills.
Excellent verbal and written communication abilities.
Ability to work independently and as part of an interdisciplinary team.
Strong organizational and time-management skills.
Compassionate and empathetic approach to patient care.
Proficiency with electronic medical records (EMR) systems.
Knowledge of hospice philosophy and end-of-life care principles preferred.
Physical Requirements
Ability to travel to patient homes, facilities, and community locations. Ability to sit, stand, walk, bend, and lift up to 25 pounds occasionally. Ability to utilize a computer, tablet, and other office equipment. Work Schedule
