Performing history and physical examinations Documenting visits in electronic medical record in a timely manner Educating patients on their health conditions in a way they can understand. Following up with patients more frequently for effective condition management. Utilizing your support team to complete standing orders Ordering diagnostic tests based on assessments, developing treatment plans, prescribing drugs and other therapies as needed Making referrals as necessary, consulting with supervising medical provider when appropriate. Maintaining patient confidentiality Participating in meetings Working collaboratively with other members of the medical team Collaborating with the management team on quality improvement initiatives Having an open mind to feedback, different perspectives, reflection, and growth.
Must have completed an accredited NP/PA program and be registered by the NYS Department of Education. 3 years or more experience practicing as a Nurse Practitioner or Physician assistant Experience working with a diverse and under-served patient population a plus Proven ability to work in a team environment eClinicalWorks experience is a plus

$140,000.00 - $170,000.00 per year (plus performance based bonus)
