The Registered Nurse Case Manager is responsible for the delivery and coordination of the Plan of Care for hospice patients. This includes performing initial evaluations, assessing patient needs every 15 days, and collaborating with the interdisciplinary group.
Candidates must hold an unrestricted RN license and have at least one year of clinical experience (two years for Louisiana). A valid driver's license, vehicle insurance, and access to a dependable vehicle are required.
Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition Documents problems, appropriate goals, interventions, and patient/family response to hospice care Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily

Preferred Qualifications: Current CPR Certification or ability to complete within 90 days of hire
