Job description

Position Description

The Referral Specialist is to assist in care coordination of services as it relates to referrals generated by medical provider. The Referral Specialist is responsible for preparation of the referrals and ensures all necessary clinical information is included/attached, making appointments with referral sources on behalf of the patient.

Essential Job Functions

The following job duties are mandatory requirements of the Referral Specialist

  • Maintain professional demeanor always, including demonstration of sound moral and ethical character, compassion, adherence to all agency standards for dress and decorum
  • Complies with HIPAA regulations
  • Protecting the safety and integrity of patient related information in compliance with agency protocol, i.e. not sharing personal passwords.
  • Demonstrates proficiency in use of electronic medical records.
  • Reviews the referrals that have been generated daily
  • Organize the referrals to include all necessary clinical information
  • Schedule appointments on behalf of the patient to ancillary and specialty services or social based services as deemed necessary
  • Verifies insurance coverage and obtains required authorizations
  • Maintains pending referral list in the EHR
  • Retrieves outstanding consultation reports/ diagnostic imaging reports from external agency.
  • Maintains patient portal requests from client and assigning to appropriate staff.
  • Facilitates and participates in care coordination as appropriate for patient, reporting external referral appointments made for the client to other programs including but not limited to ADHC, Health homes and case management
  • Attends Care team huddles to collaborate with the care team members at the discretion of the provider, to optimize quality care.

Other Responsibilities

The following duties are to be performed as assigned by the supervisor/provider

  • Assist client with transportation schedule to referral agency if necessary
  • Conducts reminder calls to patient regarding upcoming appointments scheduled to external referral agency.
