Conduct community outreach and provide public education on chronic disease prevention, immunizations, and STI/HIV prevention. Establish partnerships with healthcare providers and community leaders to address vaccine hesitancy in high-risk populations.
Requires a high school diploma or GED, along with four years of health/social services experience or a Texas CHW certification. A Clinical Medical Assistant certification is mandatory for vaccine administration.
Conducts community outreach and provides education to the public on health issues like chronic disease prevention and management, immunizations, and STI/HIV prevention. Works with high-risk populations in areas with low vaccination rates to address vaccine hesitancy, target populations will include individuals from various socio-economic and cultural backgrounds and areas identified as high socio-economic needs. Establishes and maintains relationships with various agencies including health care providers, community leaders, schools, and worksites to facilitate on-going education and outreach efforts. Develops and distributes promotional and educational materials and resources.
