$41.00 per hour | Base rate + visit differentials What You Will Do Evaluate, treat, and provide speech‑language therapy to home health patients using appropriate therapeutic techniques. Educate patients and families on treatment plans, maintain clinical documentation, and follow performance improvement guidelines. Support discharge planning and connect patients with appropriate community or support programs. What You Will Need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required. Speech-Language Pathologist credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required.
