Ponte Vedra Beach, FL 32082 Home Health Aide (CNA) Ponte Vedra Beach, FL | Onsite About This Role At Vicar's Landing, our Home Health Aides play a vital role in helping residents live safely, comfortably, and independently. This position provides compassionate, person-centered care both within our community and in residents' homes, supporting their physical, emotional, and social well-being. Working under the direction of nursing leadership, you'll assist with daily living activities, monitor resident well-being, and help create meaningful experiences that enhance quality of life. Your care, kindness, and dedication make a lasting impact every day. What You'll Do Provide routine nursing care and assistance in accordance with residents' individualized care plans. Assist with resident admissions, transfers, and discharges. Support activities of daily living while promoting residents' comfort, safety, and independence. Observe, document, and communicate changes in residents' condition to the nursing team. Complete required documentation accurately in electronic medical records and daily flow sheets. Assist with meal service and other resident support activities as needed. Maintain a clean, safe, and sanitary environment consistent with healthcare standards. Participate in training, in-services, and team meetings to support continuous learning and quality care. QualificationsSkills You Need Must Have Skills Active Certified Nursing Assistant (CNA) certification. Active CPR Certification. Compassionate and service-oriented approach to resident care. Strong interpersonal and communication skills. Ability to build trusting relationships with residents, families, and team members. Ability to accurately document care and follow established procedures. Strong observation skills and attention to detail. Ability to work independently and manage priorities effectively. Ability to safely assist residents with transfers, mobility, and activities of daily living. Ability to lift, transfer, push, or support residents as required, including lifting up to 50 pounds. Ability to maintain resident confidentiality and professional standards. Valid driver's license and acceptable driving record if transportation duties are assigned. Nice to Have Skills Previous experience in home health, skilled nursing, assisted living, memory care, or senior living. Experience with electronic medical record (EMR) systems. Knowledge of dementia care and aging-related health conditions. Experience providing transportation assistance and companionship services.

Requirements

Requires a high school diploma or GED and 12 to 18 months of related experience or specialized training. Must be physically capable of lifting up to 100 pounds and pushing a wheelchair.
