The RN Care Coordinator collaborates with high-risk patients, families, and providers to manage chronic illnesses through a team-based approach. Daily duties include conducting patient meetings, performing documentation research, and coordinating care via various communication channels.
Candidates must be a Registered Nurse with a degree in nursing or a related science field and 5-10 years of diverse clinical experience. Proficiency in Microsoft Office and strong organizational and communication skills are required.
Partnership Health Center in Somerville, NJ is seeking a full-time RN Care Coordinator to work on-site at our Somerset Partnership Health Center. The position is Monday-Friday and pays competitively. The Integrity Health Care Coordinator works in collaboration and continuous partnership with chronically ill or “high-risk” patients, their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach. A rewarding and challenging position, the successful candidate will meet the following criteria:
A typical day for the Care Coordinator will entail spending half of the day conducting extended patient meetings or other engagements as described above (approximately 30-45 minutes long). The other half of the day will be spent on follow-up with patients, family/caregiver(s), providers, and community resources via secure email, phone calls, text messages, and other communications.
5-10 years experience in a variety of settings including Hospital, Ambulatory Care, Post-Acute Care, Health Plan/Benefits Management, and Disease Management
