Review skilled nursing visit notes (RN/LPN). Review skilled nursing evals (RN). Review skilled therapy visit notes (PT/OT/ST). Review skilled therapy eval/re-evals (PT/OT/ST). Review all skilled discharge summaries. Review all discipline case conference notes. Review all OASIS types. Perform all other duties as assigned.
This is a desk position that involves sitting, standing, stooping, and walking as well as an inordinate amount of telephone communication. This position requires minimal lifting. One must be able to adequately hear on the telephone, with no more than an amplifier and be able to communicate both verbally and in writing, in English.
High School Diploma, (or GED or High School Equivalence Certificate). Must be literate and able to maintain simple records, in English. Must have a criminal background check. Homecare Clinical Specialist - OASIS (HCS-O) certification a plus but not required.
