About the role

The Community Case Manager works directly with children, youth, and families to connect them with essential resources. They are responsible for documenting client progress and ensuring follow-through on treatment plans and referrals.

The pay for this position is $44,850-$58,909 annually.

An Associate’s Degree and two years experience and/or Bachelor’s Degree.

Requirements

Candidates must have a Bachelor's degree or an Associate's degree with two years of experience. Experience in the mental health field and the ability to provide holistic case management are required.
