Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs Manage emergency situations in a prompt, precise, and professional manner Complete medication administration and treatment needs for all assigned patients Provide nursing care in accordance with applicable regulations and Company policy Prepare and maintain accurate and complete clinical and progress notes on a timely basis
