Competitive Pay: $42.25-$45.03/hour + credit for experience Schedule: This is a part time position with varying schedules | On call rotation required Supportive Team: We value our team members as much as our clients What You'll Do: Assess client needs and develop individualized care plans to support optimal health outcomes Coordinate with physicians, therapists, and caregivers to implement and monitor care plans Adjust care strategies as needed based on client progress and changing needs Educate clients and families on disease management, medications, and care best practices Maintain accurate records and ensure compliance with all medical standards and regulations What You Will Need: Active MN Registered Nurse License.

Experience in home health, case management, or clinical care (Preferred).
