About the role

Position Summary

The Speech-Language Pathologist (SLP) evaluates, develops, and implements individualized treatment plans for residents in a Skilled Nursing Facility (SNF) with communication, cognitive-linguistic, voice, and swallowing disorders. The SLP works collaboratively with the interdisciplinary care team to maximize residents' functional independence, safety, quality of life, and ability to participate in daily activities. Position Responsibilities: Evaluate and treat residents with speech, language, cognitive, voice, and swallowing disorders. Complete evaluations, treatment plans, progress notes, and discharge documentation. Assess and manage swallowing disorders, including diet and safety recommendations. Develop and update individualized treatment plans based on resident needs and goals. Participate in care conferences and interdisciplinary team meetings. Collaborate with physicians, nursing staff, therapists, dietitians, and other healthcare team members. Educate residents, families, and staff on treatment recommendations and swallowing precautions. Provide in-service training and education to facility staff. Monitor resident progress and adjust treatment plans as needed. Support discharge planning and transitions of care. Maintain compliance with facility policies, regulatory requirements, and documentation standards. Maintain treatment equipment and follow infection control practices. Perform other duties as assigned. Position Requirements: Professional degree in speech language pathology required. Masters degree in speech pathology from an accredited program. Eligible for Speech-Language Pathologist license to practice in the state of WI. Must hold and maintain Wisconsin Speech-Language Pathologist license. Must maintain a valid, current driver's license. Must maintain BLS certification. 1+ years of related work experience preferred. Experience in a hospital setting a plus. Exceptional accuracy and attention to detail required. Knowledge, Skills, & Abilities Intermediate proficiency with computers is required. Strong assessment and planning skills. Strong quantitative and analytical competency. Self-starter with excellent interpersonal communication and problem-solving skills.

Requirements

A Master's degree in speech pathology and a valid Wisconsin SLP license are required. BLS certification and a valid driver's license are mandatory, with one or more years of experience preferred.
