As an Integrated Care Specialist, Licensed Social Worker, you’ll play a meaningful role in providing person-centered, trauma-informed support and coordination of services for individuals with intellectual and developmental disabilities (IDD), including those with co-occurring behavioral health needs (e.g., ASD, mental health conditions). This role focuses on improving functioning across life domains including behavioral health, adaptive skills, communication, and community integration.
• Experience working with people with autism spectrum disorders and developmental disabilities preferred.

Requires a Bachelor's degree in social work and licensure as a Licensed Social Worker (LSW) in the State of Ohio. Candidates must have experience with mental health conditions and possess a valid driver's license with a personal vehicle.
