Provides psychosocial assessments, counseling, and advocacy services to patients in Pediatric Endocrinology and CF programs. Coordinates community resources and supports families dealing with complex or catastrophic care needs.
Overview Provides assessment, counseling, information and referral, advocacy services, and other resource assistance to patients directly or through consultation. Would work with our Pediatric and Adult CF programs and give support to our Complex Primary Care Department.
Provides complete psychosocial assessments of patients and families. Provides counseling to individuals and families to help solve social and emotional problems. Coordinates community resources to address/meet identified needs. Assists families in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers and linkage to resources outside the community. Provides consultation, assessment, referral and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence. Organizes support groups. Documents objective, concise and pertinent clinical data in the medical record. Perform other duties as assigned.
Master’s Degree in Social Work (MSW) required. Social work experience in an acute, home care, or ambulatory setting preferred. Strong organizational skills, verbal and written communications skills. Sound decision making and judgment. Time management skills. Negotiation skills. Proven ability to deal with conflict resolution. Comfortable with basic computer skills. Required Licensure/Certifications None
