  • Duties/Responsibilities: Oversees patient cases from admission to death/discharge. Completes all nursing assessments for each patient assigned. Responsible for maintaining patient medication profiles, updating as needed, while ensuring updated profile is in patient charts at the office, as well as in the facility or home. Updates the plan of care for each assigned patient every two weeks. Completes the re-certification process by assessing and accurately documenting findings on appropriate forms. Makes RN supervisory visits every 14 days on all assigned patients. Completes admissions as assigned. Provides on-call hours as determined by Patient Care Manager. Accurately communicates patients’ information to Patient Care Manager and medical staff. Participates in sharing of knowledge through in-services, staff meetings, and assisting in orientation of new personnel. Performs routine duties without physical supervision. Recognizes and responds to priorities in the patient care process. Makes sound judgements based on factual information, while seeking guidance and direction as needed for performance of all duties. Provides support to patients and families in a courteous manner with a positive tone. Assesses, plans, implements, and evaluates patient care. Provides nursing care to patients according to established policies and procedures. Checks status of medications provided by Hospice at each visit, and reorders as needed. Takes and records vital signs of the patient in an accurate and precise manner. Reports and records any changes in condition or behavior of the patient. Communicates concerns and/or changes to all disciplines. Any and all other duties and responsibilities as assigned. #HOSPICE
  • QualificationsRequired
  • Skills/Abilities: Excellent verbal and written communication skills. Excellent interpersonal and customer service skills. Excellent organizational skills and attention to detail. Excellent time management skills, with a proven ability to meet deadlines. Strong analytical and problem-solving skills. Ability to function well in a high-paced, and at times, stressful environment. Proficient with Microsoft Office Suite or related software.

Requirements

Requires a current Registered Nurse license, CPR certification, and a 4-year degree. Candidates should have 1-2 years of experience in home health, hospice, or long-term care facilities.
