About Duke HomeCare & Hospice Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, as well as serves as the home forthe Duke Caregiver Support Program.

Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC.

Relocation Assistance (based on eligibility)

At Duke Home Health, you are able to meet patients where they are! By working as a home health nurse, you are able to help keep patients out of the hospital and in their homes where they are most comfortable. Duke HomeCare and Hospice provides innovative and thoughtful care by using an interdisciplinary team approach to achieve the best possible outcomes for patients, families and the communities for which they serve. In this role, you would have the opportunity to connect the patient with the right level of care at the right time.

Shift

  • (5) 8 hour days: Monday to Friday.

  • Summary: As a Home Health Licensed Practical Nurse with Duke University Health System, you will make providing service your priority while caring for the whole person in a patient-centered atmosphere. The Home Health Licensed Practical Nurse assume responsibilities for direct nursing care of assigned patients under the supervision of a registered nurse or physician in patient care area. Provides nursing services to patients and families in accordance with the scope of the LPN as defined by the North Carolina Board of Nursing. Knowledge of scope of licensed practical nurse, ability to delegate to nursing support staff. Considerable knowledge of the care and treatment of patients and special procedures that apply to practical nursing. Able to independently seek out resources and work collaboratively. Able to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others. Able to teach patients and families in accordance with the nursing plan of care. Able to use sensory and cognitive functions to process and prioritize information, treatment, and follow-up. Competent in BLS and/or other specialized life support requirements designated by work area or organization. Able to use fine motor skills. Able to record activities, document interventions. Able to use computer and learn new software programs. Able to remain focused and organized.

  • Required: Work requires graduation from an accredited Practical Nurse program. Highly preferred experience in Home Health Setting for 1+ years, or high acuity experience to include wound vac experience. Degrees, Licensure, and/or Certification Current State of North Carolina LPN licensure is required or Multi-State Compact License (If out of state compact license, must obtain permanent NC license within 60-days of employment). Active and current driver's license and automobile insurance BLS Certification is required (If expired, this training will be provided during orientation.)

Requirements

Requires graduation from an accredited Practical Nurse program and a current North Carolina LPN license or Multi-State Compact License. BLS certification and a valid driver's license with automobile insurance are mandatory.
