How you’ll make an impact in this role Oversee primary needs of patient, including assessment of client and family/caregivers, and develops comprehensive plan of care. Provide primary home care/consultation to complex client and family needs within area of expertise and in collaboration with the physician. Monitor patient care through assessments, evaluations and/or patient records..
8:00 a.m.- 4:30 p.m, every Friday, every other Saturday, rotating Sundays and weekdays.
Counselor credentialed from the Illinois Department of Financial and Professional Regulation preferred. Social Worker credentialed from the Illinois Department of Financial and Professional Regulation preferred. Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
What additional preferences we're seeking Master's Degree required Behavioral health inpatient experience HIGHLY preferred Must have weekend availability
