Oak Bridge - Ponte Vedra Beach, FL 32082The primary purpose of this position is to provide direct nursing care to residents under the direction and supervision of a Registered Nurse (RN) or Director of Nursing Services (DNS). The Licensed Practical Nurse (LPN) delivers nursing care in accordance with physician orders, facility policies, and applicable federal, state, and local regulations. The LPN supports resident safety, comfort, dignity, and quality of life while maintaining accurate clinical documentation and effective communication with the interdisciplinary care team. WORK ENVIRONMENT Vicar’s Landing Oak Bridge Memory Care is a MontessoriInspired Lifestyle Memory Care community dedicated to creating a supportive, engaging, and purposeful environment for residents living with memory impairments, including dementia and Alzheimer’s disease. The community is committed to providing highquality, personcentered care that aligns with Montessori principles. In this environment, team members support residents in maintaining their independence, dignity, and sense of self through meaningful activities, individualized care approaches, and respectful daily interactions. ESSENTIAL DUTIES & RESPONSIBILITIES Provide direct nursing care to residents in accordance with physician orders, established nursing practices, and facility policies and procedures. Administer medications and treatments as prescribed and document administration accurately and timely in the electronic medical record. Monitor residents for changes in condition and promptly report observations, concerns, or changes to the RN or Director of Nursing. Assist with the development, implementation, and updating of resident care plans by providing input related to resident conditions and response to care. Perform routine nursing procedures, including wound care, catheter care, specimen collection, and monitoring of vital signs. Maintain accurate, complete, and timely clinical documentation in accordance with facility charting standards. Provide guidance and direction to Certified Nursing Assistants (CNAs) to ensure resident care needs are met safely and appropriately. Assist with admissions, transfers, and discharges by completing required assessments, documentation, and communication. Ensure compliance with infection control practices, safety procedures, and emergency protocols. Support resident rights and promote dignity, privacy, and respect in all aspects of care delivery. Communicate effectively with residents, families, nursing staff, and interdisciplinary team members to promote continuity of care. Assist in maintaining adequate supplies and equipment on the unit and report shortages or concerns to the RN or supervisor. Participate in required training, inservice education, and emergency preparedness drills. Perform other related duties as assigned or required. Qualifications Must possess a valid and current Licensed Practical Nurse (LPN) license in the State of Florida. Must possess a CPR/BLS certification. Must be able to read, write, and speak English fluently. Experience Minimum of one (1) year of nursing experience in a hospital, longterm care, memory care, or healthcare setting preferred. Knowledge of nursing practices, medical procedures, and applicable laws and regulations related to longterm care. Ability to work collaboratively with RNs, CNAs, and interdisciplinary team members. Skills, Abilities & Competencies Strong clinical and observational skills Effective verbal and written communication Ability to follow direction while exercising sound nursing judgment Compassionate, residentcentered approach to care Ability to manage multiple tasks in a fastpaced environment PHYSICAL REQUIREMENTS Ability to move intermittently throughout the workday. Ability to communicate clearly and effectively in English. Ability to cope with the mental and emotional demands of the position. Sufficient vision and hearing (with or without corrective devices) to safely perform job duties. Ability to function independently with flexibility and professionalism. Ability to work effectively with residents, families, staff, and external agencies. Ability to work with ill, disabled, elderly, emotionally upset, and occasionally uncooperative individuals. Ability to lift, push, or pull up to 50 pounds. Must be free of communicable diseases as required by applicable regulations.

Requirements

Must hold a valid Florida LPN license and CPR/BLS certification. A minimum of one year of nursing experience in a healthcare or long-term care setting is preferred.
