Join Rising Star Home Care Services as a Registered Nurse (RN) specializing in Home Care Client Assessments. This per diem position offers the flexibility to provide compassionate care in the comfort of our clients' homes in Elkton, MD.
Conduct comprehensive assessments of clients' health and home care needs. Develop and implement individualized care plans in collaboration with clients and families. Monitor patient progress and adjust care plans as necessary. Provide education and support to clients and their families regarding health management.
