Duties and Responsibilities

The Family Support Navigator (FSN) is an active member in the FLACRA team and provides peer support services to family members of individuals with serious mental illness and or substance use. Under the supervision of the Program Manager the FSN will function as a role model to peers, family members and the community exhibiting competency in personal recovery and use of coping skills; serve as a consumer advocate, providing consumer information and peer support for clients in outpatient and inpatient settings. The FSN will have flexible working hours that will include evenings and weekends to meet the needs of the families in the community. FLACRA will use a holistic and integrated approach to address the needs of families and loved ones struggling with addiction, helping families navigate the complex barriers frequently encountered when engaging in addiction treatment and recovery. Duties Serves as an active member of the client’s treatment team, including participation in team meetings and treatment planning meetings. Assists clients in development of recovery plan, including crisis plan. Implements and coordinates recovery plans for individual clients in assigned population group. Provides case management, resourcing, service collaboration for individual clients as extension of case management services. Maintains accurate, thorough records on all assigned cases and uses documentation to assist clients in evaluating progress. Provides advocacy services on behalf of clients. Informs supervisor, and other members of treatment team as indicated, of issues/concerns related to individual clients. Anticipates in required trainings to maintain certification. Knowledge of Crisis Intervention and Early Engagement. Assist individuals and their loved ones to become engaged in recovery-focused activities through guidance, education, and support regarding addiction and opioid abuse treatment, understanding the progression and impact of addiction, finding support groups, cultivating peer interactions, and engaging in additional treatment and community services. Cultivates relationships with local addiction prevention, treatment and recovery services, managed care organizations, local substance use disorder councils, coalitions and community stakeholders to assist families with accessing local treatment and support services. Demonstrates teamwork and an ability to work in a team environment with coworkers and supervisors. Demonstrates the ability to be flexible and cooperative. Responds appropriately to crisis/emergency situations in order to ensure/secure the safety/well-being of the clients. Assures all documentation is completed in a timely and accurate manner in compliance with agency/OASAS regulations, practices, policies & procedures. Demonstrates professionalism by providing quality service and care with respect and dignity for the clients and as a representative of FLACRA. Provides back up support in times of crisis/urgency to other COTI team members Provides 1 week of overnight on call services in rotation with others. Tier 2 and 3 staff are expected to model leadership and help support Tier 1 peers. Perform other duties and functions as directed. QualificationsMinimum Qualifications: Completion of NY state-certified peer support specialist training through OMH or OASAS. Peer Support Specialist Certification. NYS Driver’s License required. CRPA-F strongly encouraged within first year of hire. Tier 1 – Participation in first offered peer certification training, acquiring CRPA-P no later than 4 months after employment. Tier 2 – certification as a CRPA-P or CRPA Other Skills/Knowledge and Experience High School Diploma or GED. Eighteen years of age or older. Good interpersonal skills and written and oral communication skills. Knowledge of applicable population group and principles/philosophy of recovery. Ability to multi-task, have good problem solving and time management skills and the ability to remain calm in a crisis. Demonstrated ability to communicate effectively and work cooperatively with culturally diverse persons, staff and community service providers. Must have NY state driver license. Comfort/ability to provide services in the community to those in need. Ability to provide overnight on call coverage in a weekly rotation.

Requirements

Requires a High School Diploma or GED and NY state-certified peer support specialist training through OMH or OASAS. A valid NYS Driver's License is mandatory, and CRPA certification is strongly encouraged.
