Provide home-based care and assessments for patients with illnesses or chronic diseases following hospital discharge. Coordinate with physicians and oversee support staff while educating patients and families on care plans.

Compensation: $50-$150 per visit (dependent on visit type), PLUS mileage and travel compensation

Must hold an active Colorado RN license and be a graduate of an accredited nursing program with at least 1200 hours of RN experience. CPR/BLS certification and the ability to pass a background check and drug test are required.
