The NP or PA will provide comprehensive primary care, including diagnostic testing and pharmacologic treatments, within a collaborative care team. They will also implement population management strategies to improve outcomes for patients with chronic illnesses.
Family Medicine is located within the Baldwin Building, NW Clinic, NE Clinic, SE Clinic, and Kasson Clinic. The exact practice location will be determined based on practice needs at the time of hire.
The NP or PA will work in a care team setting in collaboration with other team members including physicians and other allied health staff. The NP or PA will be involved in population management strategies to improve chronic illness outcomes. The provision of care includes but is not limited to comprehensive evaluation, ordering and interpreting diagnostic tests, prescription of pharmacologic agents and treatments, non-pharmacologic therapies, and counseling patients.

Candidates must be a certified Nurse Practitioner (FNP) or Physician Assistant with valid Minnesota licensure, BLS certification, and DEA registration. Experience in primary care and treating both pediatric and adult patients is preferred.
