The Collaborative Care Behavioral Health Provider provides services alongside primary care providers in a manner that reinforces behavioral health care as part of Gundersen Health System’s approach to working with patients to achieve the best possible level of medical, mental and behavioral health.
Education and Learning: Required
Masters of Science in Social Work or a comparable degree in mental health based on a clinical curriculum that meets State of Wisconsin Licensure and Provider Status Approval requirements.
