Baywood Home Care is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate care, support patients and families, and help individuals maintain their independence in the comfort of their own homes. Apply today and receive a response within 48 hours.

  • Conduct patient assessments and develop individualized care plans
  • Coordinate services with physicians, therapists, caregivers, and community resources

  • Maintain accurate clinical documentation and regulatory compliance

Qualifications

  • Current Minnesota Registered Nurse (RN) license required
  • Home health, case management, transitional care, or related clinical experience preferred
  • Strong assessment, communication, and care coordination skills
