We specialize in the treatment of chronic wounds and provide comprehensive wound management services in patients’ homes, assisted living facilities, group homes, and other community settings.
We are seeking an experienced and compassionate Wound Care Nurse Practitioner to join our team and provide mobile wound care services throughout Brooklyn, Queens, and occasionally Manhattan.
The Wound Care Nurse Practitioner will independently evaluate, diagnose, and manage patients with acute and chronic wounds in home and community-based settings. The ideal candidate is comfortable working autonomously, possesses strong clinical assessment skills, and has experience managing complex wounds including pressure injuries, diabetic ulcers, venous ulcers, arterial ulcers, surgical wounds, and other chronic wounds.
The provider will be expected to complete approximately 20–30 patient visits per week.
Candidates with experience performing wound care procedures are strongly preferred, including:
Sharp/excisional wound debridement Selective debridement Incision and drainage Skin substitute application Negative Pressure Wound Therapy (Wound VAC) management Compression therapy application Tissue biopsy
Certified Wound Care Nurse Practitioner (CWCN, WCC, CWSP, or similar certification). Experience performing bedside wound procedures. Bilingual Spanish-speaking candidates strongly preferred. Experience using eClinicalWorks (eCW) or similar EMR systems. Compensation & Benefits

Requires an active New York State Nurse Practitioner license, National Board Certification, and DEA registration. A minimum of 2 years of wound care experience and a valid driver's license for travel within NYC are preferred.
