The Social Worker II role within the University Hospitals offers a unique chance to become part of a dynamic, collaborative interdisciplinary team. This position adds value to patient care and significantly assists individuals with various issues. We currently offer an opportunity for a PRN Social Worker to support patients with mental health issues. A Brief Overview Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital. What You Will Do Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).

Provides age specific care to patients/families in all age groups. Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.

Develops plans with attention to individual patient and family goals

Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies. Communicates/collaborates with the work team using a multidisciplinary approach. Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers. Assists with recruitment, and orientation/mentoring/education of new staff and or social work interns. Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.

Education Master's Degree from an accredited school of Social Work (Required) Work Experience Hospital/Health Agency experience. (Preferred) Knowledge, Skills, & Abilities Computer skills (Required proficiency) Theoretical and clinical knowledge regarding health care delivery (Required proficiency) Multi-tasks and prioritizes work (Required proficiency) Works autonomously (Required proficiency) Communicates effectively with persons of various backgrounds (Required proficiency) Licenses and Certifications Licensed Social Worker (LSW) (Required) Licensed Independent Social Worker (LISW) (Preferred within one year of eligibility) Position Details/Preferred Qualifications The Psych Emergency Access Center is the evaluation and transfer center for the psychiatric population in all of our entity emergency departments.

The Social Worker performs direct evaluations to determine admission or discharge.

Patients are seen via in-person and telemedicine.

It is a very fast-paced environment as patients from all entities are seen within one hour of referral time.

The ideal candidate will be autonomous, reliable, a critical thinker, have a crisis background, have strong passion for the psychiatric population, and be a self-starter.

The position requires someone with availability during the week and on weekends to cover days and nights.

Shifts are typically 12 hours; requirements are 4 shifts every 6 weeks.

It is both telemedicine in Shaker Heights, and in-person at our Cleveland Medical Center.

Requirements

Requires a Master's degree in Social Work and a Licensed Social Worker (LSW) certification. Preferred candidates have hospital experience and the ability to obtain LISW certification within one year.
