About the Practice At Advanced Primary Care, we specialize in patients who are 65+ with multiple chronic conditions like diabetes, hypertension, heart failure and COPD. Join our multidisciplinary team consisting of physicians, APPs, nurses, pharmacists, social workers and MAs. We collaborate together to deliver innovative and quality care.

There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area.
Scope of work
Serves as a subject matter expert in the practice.
Trains new staff.
Performs a variety of medically related and clinical tasks by assisting providers in providing direct patient care.
May perform moderately complex procedures within scope of certification.
