We are hiring a Certified Nursing Assistant for a Home Health position providing care to patients in their homes in the Chicago, Lincolnwood, Evanston and surrounding areas.
Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
Work independently, but never alone
Travel to patient homes within assigned territory
