The physician will join a multi-disciplinary team in an outpatient setting to provide specialized care for patients with Type-II Diabetes. Responsibilities include collaborating with NPs, pharmacists, and educators to improve patient outcomes and quality of life.

Highlights Community Physician Network is seeking an Internal Medicine physician or Endocrinologist with special interest in Type-II Diabetes management to join our new Diabetes Care Clinic in Kokomo, Indiana.

The physician in this role will join a multi-disciplinary team (NP, pharmacy, social work, Diabetes educator) in an outpatient setting.
