Current nursing license in the State of Pennsylvania Minimum of two [2] years' experience in patient care Monday-Friday 6am-2:30pm Wound care certified PCC experience On Call rotation
1. Provides skilled nursing care that includes activities such as teaching, treatments, including IV therapy, venipunctures, complex wound care and health promotion practices 2. Carries out preventive, rehabilitative and restorative nursing procedures appropriate for the patient’s care and safety. 3. Assesses signs and symptoms in conjunction with reporting reactions to treatments, including drugs, and changes in patient’s physical or emotional condition to the physician 4. Develops the plan of care cooperatively with the physician, patient and family with revisions made based on patient condition 5. Instructs and counsels the patient and family members regarding the care requirements of the patient in the home 6. Obtains a medical history, particularly as it relates to the present condition, from the patient and/or family member at the start of care. 7. Conducts a brief physical examination and nursing assessment of each patient 8. Coordinates total patient care, serving as the primary nurse for designated patients and evaluating the effectiveness and the appropriateness of care 9. Completes daily med pass
