Comprehensive benefits beginning day one (Medical, Dental, Vision, PTO, Life Insurance, STD/LTD, and more) Retirement savings plan with employer match Generous paid time off program plus 7 paid holidays No mandatory overtime Employee referral incentive program Access to state-of-the-art equipment, unlimited CEUs, and a supportive team-focused work environment What You Will Do Manage a social work caseload, supporting patients and families experiencing social, emotional, or environmental challenges that impact their health Collaborate closely with physicians, nursing staff, hospital personnel, and community agencies to ensure coordinated, high‑quality care Participate in case conferences to assess social needs, family dynamics, strengths, and barriers to care Advocate for patients by connecting them to community resources and supporting transitions across care settings Help patients and families understand and follow medical recommendations to support optimal health outcomes Maintain accurate clinical documentation and provide timely progress updates to physicians and care teams Serve as a resource and educator to clinical staff on how emotional and social factors affect physical health Build and maintain relationships with external clinicians and community partners to meet patient needs Actively promote excellent customer service, patient safety, and organizational values What You Will Need Master’s degree in Social Work (MSW) 1-2 years recent social work experience with Psych background strongly preferred. Active and valid Indiana social work license Minimum of one year of social work experience in a medical, healthcare, or mental health setting Strong clinical judgment, critical thinking, and problem‑solving skills Ability to communicate clearly and effectively with patients, families, clinicians, and community partners Comfort working collaboratively in a multidisciplinary healthcare environment Basic competency with electronic medical records and computer applications Our Commitment Rooted in our Mission and Core Values, we honor the dignity of every person and recognize the unique perspectives, experiences, and talents each colleague brings. Requirements

Requires a Master's degree in Social Work and a valid Indiana social work license. Candidates should have at least one year of experience in a medical or mental health setting, with a psychiatric background strongly preferred.
