SUMMARY

The Social Worker provides clinical and supportive services to senior residents in a supportive housing setting to promote housing stability, wellness, independence, and quality of life. The Social Worker delivers person-centered, trauma-informed, and strengths-based services, including psychosocial assessments, crisis intervention, care coordination, and short-term counseling. The role supports residents who may be experiencing chronic medical conditions, behavioral health challenges, cognitive decline, social isolation, and histories of homelessness or housing instability.

Duties & Responsibilities

  • Must be able to independently assess the psychosocial functioning and needs of clients/household and contribute to the formulation and implementation of a support plan identifying the challenges, strengths, coping skills and assistance needed, in collaboration with the client and interdisciplinary team.
  • Must be able to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.
  • Must have knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures.
  • Must be able to provide consultation services to other staff about the psychosocial needs of clients and the impact of psychosocial problems on health care and compliance with treatment.
  • Must have knowledge and skill in the use of computer software applications for drafting documents, data management and tracking.
  • Provides crisis intervention and stabilization as needed. May require some after-hours and or weekend follow-up to emergency situations.
  • Identifies program needs and makes recommendations.
  • Provide monthly training to staff regarding the impact of mental health, substance use and medical barriers that residents face
  • Coordinate with medical providers, mental health professionals, home care agencies, and community-based organizations.
  • Advocate for residents within healthcare, social service, and legal systems.
  • Support hospital discharge planning to prevent readmissions.
  • Participate in case conferences and interdisciplinary team meetings.
  • Maintain accurate, timely documentation in compliance with agency and funder requirements.
  • Collaborate with the Recreational Specialist to support psychosocial programming.

Experience, education and special requirements

  • Current, active Social Work license in good standing (e.g., LMSW, LCSW).
  • Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE) or a CASAC.
  • NYS licensure with at least two years in an area of specialized social work practice.
  • Minimum 2–3 years of experience working with formerly homeless individuals, individuals in supportive housing, congregate care or residential facilities or individuals with complex medical and behavioral health needs, preferred.
  • Strong knowledge of aging-related physical, cognitive, and psychosocial conditions.
  • Experience with trauma-informed, harm reduction, and strength-based models.
  • Ability to conduct risk assessments and crisis intervention.
  • Excellent documentation and organizational skills.
  • Strong interdisciplinary collaboration skills.
  • Fluency in Spanish is highly desirable.
  • Strong knowledge of aging-related physical, cognitive, and psychosocial conditions.
