About the role

The Physical Therapist supports individuals with complex mobility and health needs through clinical assessments and the implementation of assistive technology. They collaborate with care teams and vendors to develop effective plans of care and provide instruction on equipment use and mobility.

Job Summary

The Physical Therapist - Care Delivery supports the Care Delivery team with assistive technology and rehabilitative needs for a population of individuals with complex mobility, health, and psychosocial needs.

Essential Functions

Document clinical assessment in EMR according to Care Delivery practice standards and includes documentation as required by patient’s insurers.

Participates in joint visits with primary care providers, care team members, treating therapists and vendors to evaluate patient needs and collaborate and develop most effective plan of care and to ensure appropriate adaptive equipment is in place.

Completes DME assessments and comprehensive PT evaluations, and provides instruction to patients, families, and caregivers on use of equipment, transfers, mobility etc.; Works closely with DME suppliers and manufacturers reps.

Evaluates legitimacy and coordinates cost effective solutions for DME requests and authorization review.

Responds to patient directed interventions and all communication.

Administers treatment as directed.

Attends care delivery team meetings as directed.

Provides clinical care to members via telehealth technologies (video, chat, etc.) for a clinically appropriate clinical care and care management services.

Perform any other job related duties as requested.

Education and Experience

  • Bachelor's degree in Physical Therapy or Occupational Therapy required
  • Master's degree or Doctorate in the science of Physical Therapy or Occupational Therapy preferred
  • Five (5) years of related experience required

  • Basic data entry skills and internet utilization skills
  • Working knowledge of Microsoft Outlook, Word, and Excel
  • Effective oral and written communication skills
  • Ability to work independently and within a team environment
  • Ability to work cohesively with all peers and departments at all times and even under the most stressful situations.
  • Ability to manage high volume caseloads in all settings of care (home, SNF, ALF, elderly housing, group homes, private homes)
  • Must be able to modify clinical recommendations and develop alternatives to recommended plan of care in complex cases
  • Attention to detail

Licensure and Certification

Current unrestricted license as a Physical Therapist or Occupational Therapist required

Assistive Technology Professional Certification preferred

Must have valid driver's license, vehicle and verifiable insurance. Employment in this position is conditional pending successful clearance of a driver’s license record check. If the driver’s license record results are unacceptable, the offer will be withdrawn or, if employee has started employment in position, employment in this position will be terminated

CareSource requires annual proof of Influenza vaccination for designated positions during Influenza season (October 1 – March 31) as a condition of continued employment. Employees hired during Influenza season will have thirty (30) days from their hire date to complete the required vaccination and have record of immunization verified.

Working Conditions

  • This is a mobile position, meaning that regular travel to different work locations, including homes, offices or other public settings, is essential. Will be exposed to weather conditions typical of the location and may be required to stand and/or sit for long periods of time. Must reside in the same territory they are assigned to work in; exceptions may be considered, due to business need May be required to travel greater than 50% of time to perform work duties. Required to use general office equipment, such as a telephone, photocopier, fax machine, and computer Flexible hours, including possible evenings and/or weekends as needed to serve the needs of our members
  • Over 50% (Mobile) Routine travel required

Compensation Range

$72,200.00 - $115,500.00

CareSource takes into consideration a combination of a candidate’s education, training, and experience as well as the position’s scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
