About the role

The Nurse Practitioner is responsible for the assessment and medical management of frail elderly participants. They work within an Interdisciplinary Team to oversee specialist use and inpatient care to improve quality of life.

Our program brings the region’s experts in geriatric medicine and care together to work as a team in specialized Day Centers, to help older adults enjoy the highest quality of life possible. Our wide range of services are designed to meet the varied needs of seniors, and include medical care, social services, meals, activities, transportation and much more.

Shift: Mon-Fri, 7:30am-4:00pm

Location: Bedford, PA 15522

Summary of Job Duties

Our Nurse Practitioner (NP) or Physician Assistant (PA) is responsible for periodic and episodic assessment and management of individual participants’ medical conditions, and for oversight of their use of medical specialists and inpatient care. The NP or PA is part of an Interdisciplinary Team with the knowledge and skills necessary to assess, plan care for, and provide service to frail elderly participants.

Required Education

For

NPs: Completion of an accredited, Board-approved, Master’s, Post Master’s or Doctoral Nurse Practitioner program.

For

PAs: Graduate of a physician assistant program accredited by the Accreditation Review Commission on Education for the physician assistant (ARC-PA).

Required Licensure

  • For NP: Pennsylvania RN license and CRNP certification in Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) or Family Nurse Practitioner (FNP)
  • For PA: Current state PA license certified by the National Commission on Certification of Physician Assistants (NCCPA)
  • Pennsylvania Prescriptive Authority Approval
  • DEA license
  • Valid PA Driver’s license
  • All licenses must be current, active and unrestricted

Required Experience

A minimum of 1 year providing services to a frail or elderly and/or mentally and physically handicapped population preferably in a health care setting or an equivalent combination of education and experience. A minimum of 3 years of NP or PA experience is highly preferred.

Required Skills

  • Geriatric Assessment and management of co-morbidity.
  • Effective listening and communication skills with patients, families and IDT members.
  • Clear and complete documentation.
  • Effective Team skills including but not limited to reliability, active listening, positive approaches, courtesy and respect in line with Community LIFE Mission and Values statements.

Requirements

Requires a Master's or Doctoral degree in Nursing and certifications as an AGPCNP or FNP. Candidates must hold a Pennsylvania RN license, prescriptive authority, a DEA license, and have at least one year of experience with elderly or handicapped populations.
