The RN Navigator coordinates patient-centered healthcare plans and serves as the primary point of contact for patients, caregivers, and referring physicians. They collaborate with a multidisciplinary team to identify and resolve patient needs throughout the care process.
A team-based care RN (Registered Nurse) navigator plays a pivotal role in delivering coordinated, patient-centered healthcare within a multidisciplinary team. Their responsibilities emphasize collaboration, evidence-based practice, and quality improvement. The RN Navigator is an experienced registered nurse who is accountable for the effective clinical care coordination of patients. The Navigator proactively identifies and resolves patient’s needs. and clinical guidance throughout their care within Deborah Heart and Lung Center and it's practice locations. The RN navigator coordinates a plan of care across the system for Deborah patients. The RN Navigator systematically and continuously collects and assesses information related to patient health status to develop, execute, and evaluate the plan of care for patients and/or family.
Serves as a single point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Deborah Heart and Lung Center and practice locations. Work closely with physicians, nurse practitioners, medical assistants, behavioral health specialists, and other team members to provide comprehensive care.
Required: 1-3 years Clinical experience
Preferred: experience in cardiac, critical care and or ambulatory
Required: Graduate of accredited diploma, Associates degree or baccalaureate degree in nursing
License and Credentials
Required: NJ State RN license, BLS
Cardiac-specific credentials (e.g., CCRN, CHFN), Ambulatory nursing preferred
Excellent interpersonal skills. Ability to perform well in a fast-paced complex environment. Computer literacy. Familiarity with MS Office Applications.
The minimum starting rate for this position is $44.84

Candidates must be a graduate of an accredited nursing program with a valid NJ State RN license and BLS certification. 1-3 years of clinical experience is required, with a preference for cardiac, critical care, or ambulatory backgrounds.
