Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM Rotating weekends required + 1 summer holiday & 1 winter holiday

Bonus

Up to $10,000 sign-on bonus for eligible external candidates (must be in role at time of payout; not available to current team members)

What you’ll do

As a Home Health Physical Therapist, you will deliver high-quality, patient-centered care in a home setting, helping patients improve mobility, function, and quality of life. You’ll collaborate with an interdisciplinary team while working independently across your assigned territory.

  • Key Responsibilities
  • Evaluate, develop, and implement individualized treatment plans based on patient needs and evidence-based practices
  • Monitor patient progress and adjust care plans to achieve optimal outcomes
  • Provide clear, timely, and compliant documentation to support care and reimbursement
  • Educate patients and families on treatment plans, self-care, and safety practices
  • Coordinate care with physicians, nurses, and other healthcare team members
  • Participate in case conferences, team meetings, and quality improvement initiatives
  • Support safe patient handling and promote fall and injury prevention
  • What we’re looking for
  • Strong clinical judgment and ability to manage a diverse patient population, including complex cases
  • Excellent communication and collaboration skills
  • Ability to work independently and manage time effectively in a field-based role
  • Commitment to delivering compassionate, patient-focused care

Requirements

  • Licensed Physical Therapist in Illinois (or eligible for licensure)
  • Minimum 1 year of clinical experience (or strong clinical affiliation experience)
  • Basic computer skills (documentation systems, email, etc.)
  • Valid driver’s license and reliable transportation
  • Work environment & physical demands
  • Travel required within assigned service territory
  • Work in patient homes and assisted living settings
  • Ability to lift up to 35 lbs (with assistance/equipment as needed)
  • Frequent standing, walking, bending, and assisting patients with mobility and transfers
