About the role

The Case Manager develops and revises service plans tailored to the needs of unhoused veterans while maintaining a database of client resources. The role focuses on securing stable housing and connecting clients with essential community resources.

The Case Manager is responsible for developing and revising service plans that are appropriate to the needs and preferences of the client, program requirements, funding provider standards, and VOAOHIN. The Case Manager provides appropriate, systematic evaluation and interpretive reports. The Case Manager is responsible for developing and maintaining a database of client resources.

The Richard Lugar Safe Haven program is considered a low-demand emergency shelter for unhoused veterans. Veterans are able to come to Safe Haven experiencing various issues such as mental health or substance use, and receive the help they need and deserve. Safe Haven is a housing-first program where finding adequate housing is the top priority, alongside connecting with other community resources. The Case Manager will focus on helping their veterans secure stable housing and receive resources that best meet their needs.

Qualifications: High School; Bachelors Required; Driver License; CPR

Requirements

A bachelor's degree and a high school diploma are required for this position. Candidates must also possess a valid driver's license and CPR certification.
